Can you list all the assumptions that are necessary before drawing
conclusions about future reliability from the operational history?

I think the list must contain at least:

  * the software is identical to the software for which we are
    considering the history (or it is possible to show with high
    confidence that any differences cannot affect reliability)
  * the future operating environment will be identical to the historic
    operating environment (or it is possible to show with high
    confidence that any differences cannot affect reliability)
  * the information about the history is veridical and complete.

Any more?

If these are /necessary/ assumptions, how often will they be /valid/
assumptions?
