[SystemSafety] An IEC61508 low demand SIL4 example please

Some people delight in producing doubtful mappings between standards and guidelines for software assurance saying, usually misleadingly and sometimes completely wrongly, that the 'levels' are broadly equivalent.  I find it useful to challenge these efforts by providing examples of the types of system that may be produced requiring assurance at a particular level...

So - does anyone have a real example of a system that used the Low Demand Mode SIL4 of IEC 61508?
