[SystemSafety] NTSB report on the Tesla Crash

Greetings,

The NTSB's report on the Tesla Crash is out. As with many NTSB reports, 
someone simply reading the 'probable cause' statement would reach the 
wrong conclusions about what the investigation found. The findings and 
recommendations summarize the real story. If you have time to only read 
a little bit, I suggest reading the concurring statement written by 
Member Hart and joined by Chairman Sumwalt.
